Updated CRF

The TTM2/TAME eCRF is almost complete. In preperation for the launch of the online CRF we have put together a printer-friendly version, with some small tweeks compared to the earlier CRF. The first file for the hospital stay is available here and in the updated documents section under eCRF. We will shortly add a seperate document for adverse events and follow-up. When changing to the new version please note that the daily forms refer to day 0-7 where day 0 is the time from randomisation to "break-of-day" in you local ICU. The following days should follow your regular ICU scheme for breaking down fluid balances and drug doses (e.g. 0600-0600 or 00.00 to 00.00).
